# Bariatric Surgery vs GLP-1 Drugs on Cardiovascular Events

Bariatric surgery beat GLP-1 drugs on every cardiovascular outcome in a 932,380-patient analysis. The drug side pools studies going back to 2000.

## Key takeaways

- A network meta-analysis of 43 studies and 932,380 patients comparing bariatric surgery and GLP-1 drugs.
- Surgery against controls: major cardiovascular events 0.66, heart failure 0.45, heart attack 0.53.
- GLP-1 drugs against controls: major cardiovascular events 0.85, 95% CI 0.77 to 0.94.
- GLP-1 drugs did not reach significance for heart failure or heart attack.
- The search ran from 2000 to 2024, and the authors ask for studies on newer GLP-1 drugs.

The comparison is arresting and the date range is the part to read first. This pools studies published between January 2000 and September 2024, which means most of the drug side of it is exenatide, liraglutide and dulaglutide rather than what people take now — the same problem as [reading an older trial as evidence about a newer molecule](https://thisglp.com/research/semaglutide-cochrane-review).

## What the pooled figures say

Bariatric surgery was associated with lower major adverse cardiovascular events, hazard ratio 0.66, 95% CI 0.60 to 0.74; lower heart failure, 0.45, 95% CI 0.38 to 0.53; and lower myocardial infarction, 0.53, 95% CI 0.45 to 0.63. [\[1\]](https://thisglp.com/research/bariatric-surgery-vs-glp1-cardiovascular#ref-1) GLP-1 drugs reduced major cardiovascular events, 0.85, 95% CI 0.77 to 0.94, and did not reach significance for heart failure or heart attack.

Heterogeneity ran from 66% to 93%, which the authors flag themselves while noting the direction was consistent across studies — a spread that puts the pooled numbers closer to [a summary of disagreement](https://thisglp.com/research/glp1-weight-loss-pooled-average) than to a single estimate.

Which GLP-1 drugs, and when

A study published in 2012 is not evidence about semaglutide, and a cardiovascular literature reaching back to 2000 is dominated by agents that are substantially weaker on weight and were tested in different populations. The authors close by asking for future studies on newer GLP-1 drugs, which is an acknowledgment that the arm they pooled is not the current class. Whatever the drug side of this comparison measures, it is not a measurement of what a person starting today would take.

## The other half of the comparison

Bariatric surgery has an eligibility process in front of it. Somebody has to be considered suitable, pass an anesthetic assessment, and be well enough to be operated on, and that selection happens before any outcome is counted.

A network meta-analysis links the two arms largely through their shared comparison with controls rather than through head-to-head studies, so what is being compared is partly two different groups of people — which is the standing limit of [evidence assembled indirectly](https://thisglp.com/research/do-glp1-drugs-affect-spinal-fusion-surgery).

## What the drug arm does establish

A real reduction in major adverse cardiovascular events, at 0.85 with an interval clear of one, in a pooled population of that size.

Not reaching significance on heart failure and heart attack is not the same as showing no effect on them, and with heterogeneity in this range the pooled estimates carry less than their intervals suggest. The authors’ own framing — GLP-1 drugs as an evidence-based option for people who are not surgical candidates — is the reasonable reading, and it is a clinical decision rather than [a ranking anyone shops from](https://thisglp.com/research/what-new-weight-loss-drugs-are-coming).

## Frequently asked

Is bariatric surgery better than GLP-1 drugs for the heart?

In this pooled analysis surgery showed larger and more consistent reductions. The drug arm pools studies back to 2000, so it is largely older GLP-1 agents rather than current ones.

What did GLP-1 drugs achieve?

A reduction in major adverse cardiovascular events, hazard ratio 0.85, 95% CI 0.77 to 0.94. They did not reach significance for heart failure or myocardial infarction.

Were the two groups comparable?

Not directly. Surgery candidates pass an eligibility and anesthetic assessment first, and the analysis links the arms largely through their shared comparison with controls.

How consistent were the studies?

Heterogeneity ran from 66% to 93%. The authors report directional consistency and ask for cautious interpretation.
