# Oral Semaglutide 50 mg in East Asian Adults: 14.3% Weight Loss | This GLP

What the 50 mg oral semaglutide trial in 201 East Asian adults found (14.3% against 1.3% on placebo), the conditions the headline drops, and why a storefront's 'oral semaglutide' is probably not this.

# Oral Semaglutide 50 mg in East Asian Adults: 14.3% Weight Loss

A 50 mg tablet produced a 14.3% mean weight reduction against 1.3% on placebo in 201 East Asian adults starting at 91.9 kg, at a dose several times the oral semaglutide already on the market.

Weight change, 201 East Asian adults · 50 mg daily · 14.3% · placebo · 1.3% · Mean starting weight 91.9 kg, not the 113 kg of the big trials. · A percentage of a different number is a different thing.

## Key takeaways

- Oral semaglutide at 50 mg daily produced a mean weight reduction of 14.3% against 1.3% on placebo, in 201 East Asian adults with overweight or obesity.
- 84.3% of the treated group lost at least 5% of their body weight, against 17.2% on placebo.
- Mean starting weight was 91.9 kg — well below the 113 kg of the large Western trials — so the percentages come off a different base.
- Gastrointestinal adverse events affected 63.4% of the drug arm, and 4.5% stopped because of adverse events.
- ⛔ 50 mg daily is not the oral semaglutide sold for diabetes at 7 or 14 mg, and almost certainly not what a storefront means by 'oral semaglutide'.

A tablet that works as well as an injection is the thing most readers actually want, and this is one of the trials that decides whether it exists. The answer is encouraging and comes with three conditions attached, all of which get dropped when the headline travels — the same fate as the trial figures collected in [what the semaglutide trials are worth](https://thisglp.com/research/semaglutide-cochrane-review).

## What was tested

Two hundred and one adults with overweight or obesity, with or without type 2 diabetes, were randomized 2:1 to oral semaglutide 50 mg daily or placebo [\[1\]](https://thisglp.com/research/oral-semaglutide-fifty-milligrams#ref-1). Mean age was 49, 43.3% were female, a quarter had type 2 diabetes, and mean body weight was 91.9 kg.

## What it found

Mean weight change was −14.3% on the drug against −1.3% on placebo, an estimated treatment difference of 13.07 percentage points (95% CI −15.61 to −10.52). More of the treated group reached a 5% reduction: 84.3% against 17.2%, an odds ratio of 23.00.

Gastrointestinal adverse events were reported by 63.4% of the semaglutide arm against 34.8% on placebo, and adverse events led 4.5% of the treated group to discontinue. That discontinuation figure is lower than the injectable trials generally report, though with 134 people on the drug it is not a precise estimate.

Three conditions the headline drops

The trial enrolled East Asian adults with a mean starting weight of 91.9 kg. The large Western obesity trials started around 113 kg. A percentage taken off a smaller number is not the same quantity, and setting −14.3% beside a STEP or SURMOUNT figure compares populations as much as drugs.

It is also a small trial by this field’s standards — 201 randomized, 134 on the drug — and the dose is 50 mg daily, which is several times the 7 or 14 mg oral semaglutide approved for type 2 diabetes.

## The part that matters on a storefront

That last condition is the one with commercial consequences. A seller offering “oral semaglutide” is almost certainly not offering 50 mg daily of the product this trial used, and in the compounded market the milligram is frequently not published at all — the silence counted in [who publishes a dose ladder](https://thisglp.com/research/who-publishes-a-glp1-dose-ladder).

A reader who has seen this result and gone looking for a tablet may be sold something at an unstated fraction of the dose, with no way to tell from the page. What an oral formulation does and does not deliver is covered in [what the pill actually does](https://thisglp.com/research/oral-semaglutide-what-the-pill-does), and the chemically different small-molecule tablet now in trials is a separate matter again, covered in [orforglipron, a pill without a needle](https://thisglp.com/research/what-is-orforglipron).

## What it establishes

That a peptide tablet at a high enough dose can produce double-digit weight reduction in a randomized trial. That is a real advance and it is worth stating plainly. What it does not establish is that any particular tablet a reader can buy will do it, which depends entirely on a dose most sellers decline to name.

## Frequently asked

How much weight did the tablet produce?

A mean of 14.3% against 1.3% on placebo, with 84.3% of the treated group losing at least 5% of their body weight.

Is that comparable to the injection trials?

Not directly. This trial enrolled East Asian adults starting at a mean of 91.9 kg, against roughly 113 kg in the large Western trials, so the percentages come off a different base.

Is this the oral semaglutide sold online?

Almost certainly not. The trial used 50 mg daily, several times the 7 or 14 mg approved for type 2 diabetes, and compounded sellers frequently do not publish a milligram at all.
