# 1st Optimal review

A lab-led coaching membership with GLP-1 prescribing inside it, where the membership has a price and the drug does not.

## The verdict

1st Optimal publishes $99 a month for its Weight Loss Care membership. The medication is invoiced separately, and no figure for it appears anywhere on the site. That makes the membership the only cost a reader can know before joining.

1st Optimal is a Michigan-based functional medicine service that sells monthly care memberships. Weight loss is one of several plans, beside hormone, menopause and peptide care. GLP-1 therapy is offered within that weight-loss plan.

1. ## Who writes the prescription, and what do they see?

The site states that a licensed provider reviews lab results and discusses a treatment plan after an initial consultation. A prescription, if one is written, goes electronically to a partner pharmacy. The individual prescriber is not named on the plan pages, though a nurse practitioner appears in the site's podcast.

Labs come first. The membership covers consultations, lab testing, shipping and medical supplies, plus monthly coaching on nutrition and exercise. Billing is month to month, drafted on the 1st or the 16th, with no long-term contract stated. The FAQ says pharmacy partners typically ship within 14 business days, which is slower than most mail-order GLP-1 sellers state.

2. ## What happens when the dose goes up?

The weight-loss page describes protocols that use minimum effective doses, adjusted against labs and symptoms. No dose schedule is published. Because the medication price is not published either, what a dose increase does to the monthly cost is not stated.

3. ## What the price includes

$99 a month buys the membership. Prescriptions, labs and supplements are invoiced separately, about three weeks before each refill for existing patients. The FAQ promises a reduced members-only rate on treatments but prints no rate. A total monthly cost cannot be worked out from the site.

4. ## What it claims, and what it cites

The plan pages describe GLP-1s as FDA-approved medications and list weekly injections, daily tablets and combination therapies. They do not name the molecule, say whether it is branded or compounded, or name the partner pharmacy. Compounded preparations are not FDA-approved.

## Where it holds up, and where it does not

- Strength: Month-to-month billing, no contract stated
- Strength: Labs and consultations sit inside the membership
- Strength: Coaching is part of the plan, not an add-on
- Strength: Cancellation is described step by step
- Limitation: No medication price is published
- Limitation: The molecule is not named on the plan pages
- Limitation: Shipping can take 14 business days
- Limitation: States are given only on request

## What the evidence says about this kind of treatment

The assessment above is about 1st Optimal. These are about the drugs it sells, and they are traced to the primary literature.

Right for

A reader who wants labs and coaching around a prescription, and will ask for the drug price before paying.

Wrong for

A reader comparing all-in monthly GLP-1 prices, since only the membership has one here.

## The evidence behind what 1st Optimal prescribes

These studies examine the medicines, not this provider. Results with approved products do not establish the quality or effectiveness of a seller’s compounded formulation.

- [Cochrane Review of Semaglutide for Obesity: 18 Trials Graded](https://thisglp.com/research/semaglutide-cochrane-review)

A 2025 Cochrane review pooled eighteen randomized trials. The weight figure is high certainty and smaller than the marketing, and almost everything else is graded lower.
