# FORM review

Four lines, four published figures, and not one of them is the price of a medication.

## The verdict

FORM publishes a figure on every line and a medication price on none of them. Two lines quote a self-pay care plan that FORM states excludes the medication. The other two quote an access membership, with the drug obtained through NovoCare or LillyDirect. In all four cases the number on the page buys the service, not the treatment.

FORM covers semaglutide and tirzepatide, injected and oral. All four lines are service charges. None carries a medication cost, so no line on this seller can be ranked on price here.

1. ## Who writes the prescription, and what do they see?

The $95 buys the clinical relationship. A prescriber reviews the request and writes the prescription. The manufacturer's channel fills it. FORM names those channels, which is more than most services that route a prescription do.

Two service shapes sit on one site. One is a care plan, one is an access membership. Both leave the medication to a third party and neither states its cost. That is a consistent model, and it is disclosed rather than buried. It also means the site publishes no price a buyer can budget from.

2. ## What happens when the dose goes up?

Neither shape states what happens as the dose climbs. The manufacturer sets that, not FORM. So the answer is not FORM's to give — but the pages do not say where to find it either.

3. ## What the price includes

Nothing here can be set against another seller's price, because no line states what the medication costs. The care plan figure excludes it and FORM says so. The membership figure excludes it too. A reader has to find the drug price at the manufacturer and add it themselves. FORM at least names which manufacturer, which many services routing a prescription do not.

4. ## What it claims, and what it cites

The access lines lead to the manufacturers' own channels, which dispense approved products rather than compounded ones. That is a real difference in what arrives: an approved product has passed the review a compounded preparation never enters. It also means the price is set somewhere FORM does not control.

## Where it holds up, and where it does not

- Strength: The fulfilling channel is named outright
- Strength: Membership and medication are kept distinct
- Strength: Both service shapes are labeled
- Limitation: No line states a drug cost
- Limitation: The decisive number belongs to a third party
- Limitation: Access is charged before disclosure
- Limitation: Coverage is nowhere enumerated

## What the evidence says about this kind of treatment

The assessment above is about FORM. These are about the drugs it sells, and they are traced to the primary literature.

Right for

A reader who already knows the manufacturer price and wants a named route to a prescriber.

Wrong for

A reader who needs to know what the treatment costs before committing.

## The evidence behind what FORM prescribes

These studies examine the medicines, not this provider. Results with approved products do not establish the quality or effectiveness of a seller’s compounded formulation.

- [Cochrane Review of Semaglutide for Obesity: 18 Trials Graded](https://thisglp.com/research/semaglutide-cochrane-review)

A 2025 Cochrane review pooled eighteen randomized trials. The weight figure is high certainty and smaller than the marketing, and almost everything else is graded lower.

- [Tirzepatide vs Semaglutide: Which Works Better for Weight Loss?](https://thisglp.com/research/tirzepatide-vs-semaglutide-which-works-better)

In a 72-week randomized trial at weight-management doses, weight fell 20.2 percent on tirzepatide and 13.7 percent on semaglutide. Routine-care studies found a smaller gap, three to four points.

## How FORM compares

| Seller | Who it suits | Go |
| --- | --- | --- |
| Kin Meds | Anyone building a shortlist around orforglipron, a molecule almost nobody prices | |
| Nucore Health | A reader paying from an FSA or HSA who will ask to be quoted a real figure | |
| Collective | A reader who will stay on treatment for a full twelve months, which is the term the annual membership is priced against. | |

## Frequently asked

So what is the money actually for?

The service. A care plan on two lines, an access membership on the other two. The medication is billed separately and FORM publishes no figure for it.

Can any line here be compared on price?

No. Every figure excludes the medication, so there is nothing whole to set against another seller.

Is this compounded?

The access lines are not. They lead to manufacturer channels dispensing approved products, which is a different thing entirely.
