# GOAL.MD review

Physician supervision, nutrition coaching and a phone-accessible team are included. The question is how that support translates into your own follow-up plan.

## The verdict

GOAL.MD describes more than a medication shipment: physician supervision and unlimited nutrition coaching are part of the compounded program. Before enrolling, establish who will review side effects and when you will next speak with the prescriber.

GOAL.MD offers physician-supervised compounded injections and a separate program for access to brand-name GLP-1 medicines. The two programs have different billing arrangements; a prescription depends on a medical evaluation.

1. ## Who writes the prescription, and what do they see?

A licensed U.S. physician reviews the medical history and decides whether to prescribe. GOAL.MD says it serves all 50 states, while also making the particular medication conditional on state rules, pharmacy eligibility and the clinical assessment.

The public program includes ongoing physician supervision, nutrition coaching and telephone access to the care team. Contact hours are weekdays, 9 a.m.–5 p.m. Central. A published promise of support does not establish a scheduled clinical appointment interval.

2. ## What happens when the dose goes up?

The tirzepatide page lists doses from 2.5 to 15 mg at the same price. Semaglutide also has a published dose list. The physician determines your prescription; a price table is not a reason to advance a dose before symptoms and response are reviewed.

3. ## What the price includes

Month-to-month compounded care is advertised at $179 for semaglutide and $249 for tirzepatide, including medication, supervision, coaching and shipping. The $99 and $159 headlines require 12 months paid upfront. Brand access costs $95 a month for physician care, with medication and labs separate.

4. ## What it claims, and what it cites

Its personalized [semaglutide](https://thisglp.com/research/semaglutide-cochrane-review) and [tirzepatide](https://thisglp.com/research/tirzepatide-vs-semaglutide-which-works-better) are compounded preparations, which are not FDA-approved. Brand-name access is a separate service with medication bought through a manufacturer program or participating pharmacy.

## Where it holds up, and where it does not

- Strength: Physician evaluation and nutrition coaching included
- Strength: Telephone contact hours are published
- Strength: Compounded refills require your confirmation
- Limitation: No fixed clinical follow-up interval established
- Limitation: Prepaid compounded orders become nonrefundable after payment
- Limitation: Brand access does not include the medicine

## What the evidence says about this kind of treatment

The assessment above is about GOAL.MD. These are about the drugs it sells, and they are traced to the primary literature.

Right for

Someone who values physician supervision and nutrition coaching and can confirm the clinical follow-up arrangements before ordering.

Wrong for

Someone who expects a medicine-only transaction or needs a publicly guaranteed appointment schedule.

## The evidence behind what GOAL.MD prescribes

These studies examine the medicines, not this provider. Results with approved products do not establish the quality or effectiveness of a seller’s compounded formulation.

- [Cochrane Review of Semaglutide for Obesity: 18 Trials Graded](https://thisglp.com/research/semaglutide-cochrane-review)

A 2025 Cochrane review pooled eighteen randomized trials. The weight figure is high certainty and smaller than the marketing, and almost everything else is graded lower.

- [Tirzepatide vs Semaglutide: Which Works Better for Weight Loss?](https://thisglp.com/research/tirzepatide-vs-semaglutide-which-works-better)

In a 72-week randomized trial at weight-management doses, weight fell 20.2 percent on tirzepatide and 13.7 percent on semaglutide. Routine-care studies found a smaller gap, three to four points.

## Frequently asked

Are compounded refills automatic?

GOAL.MD says they are not. The care team contacts you before renewal and charges only after you confirm.

Is the brand-name membership the same arrangement?

No. It renews automatically and covers physician services rather than the medicine. Cancellation must precede the next prescription or refill authorization.

Is GOAL.MD expensive compared with other GLP-1 telehealth sellers?

GOAL.MD publishes a starting figure of $179/mo. Across the 402 GLP-1 telehealth sellers we review that publish a single monthly figure, those run $49 to $525.42 with a median of $190, which puts GOAL.MD below the median, toward the cheaper end. The other 178 GLP-1 telehealth sellers we review quote only after an intake or price by dose, so they are not in that comparison. A starting figure is not a bill — labs, consultations and shipping are often charged separately, so confirm what your own quote includes.
