Cancer risk: what 86,000 matched records showed

Overall cancer incidence was lower among GLP-1 users, with endometrial, ovarian and meningioma all falling. Kidney cancer was the one signal pointing the other way.

Most people who look this up are looking for one thing: the thyroid warning on the label. The largest matched cohort to address cancer risk in this population does not answer that question separately, and it answers a broader one clearly. Among 86,632 matched adults with obesity, followed in electronic health records, the incidence of fourteen cancers was 13.6 per 1,000 person-years in GLP-1 users against 16.4 in non-users — a hazard ratio of 0.83 (95% CI 0.76 to 0.91) [1]. Before starting, it is worth reading alongside the gallbladder evidence, which moves in the opposite direction.

What the study found

Three individual cancers reached significance in the protective direction: endometrial at a hazard ratio of 0.75 (95% CI 0.57 to 0.99), ovarian at 0.53 (95% CI 0.29 to 0.96), and meningioma at 0.69 (95% CI 0.48 to 0.97). All three are obesity-associated, which is the most plausible mechanism — a drug that reduces obesity reducing the cancers obesity drives.

The cohort matched 43,317 users against 43,315 otherwise-eligible non-users using propensity scores, in a target trial emulation design. Mean age was 52.4 and 68.2 per cent were female.

The one going the other way

Kidney cancer came in at a hazard ratio of 1.38 with a 95 per cent confidence interval from 0.99 to 1.93, which the authors describe as a marginally nonsignificant increase. The interval touches 1.0 at its lower end, so this is not a finding of harm — it is a signal that has not resolved, and the authors call for longer follow-up to clarify it.

Reporting the overall 0.83 without this would be quoting the abstract selectively. A reader with a personal or family history of kidney cancer has a specific question for a clinician here, and most telehealth sellers publish nothing about who reviews that history — a gap counted across the market in what sellers will not tell you.

What this does not say about thyroid

Thyroid cancer is named among the fourteen studied. No separate hazard ratio for it is reported in the results, so this page does not give one. The labeled warning about medullary thyroid carcinoma comes from rodent studies and remains on the product labels; it is a contraindication for people with a personal or family history of medullary thyroid carcinoma or MEN 2, and that is a conversation for a prescriber rather than a number to look up.

How much weight to put on it

This is matched observational data, not a randomized trial, and the same caution applies that applies to the pancreatitis cohort: propensity matching cannot remove every difference between people who are and are not prescribed a drug. Cancer also takes years to develop, and a cohort drawn from 2014 to 2024 is short for that. The direction is encouraging and the follow-up is not yet long enough to be conclusive. If you are weighing it against a purchase decision, the questions sellers leave open are the practical half.
