The verdict

FORM publishes a figure on every line and a medication price on none of them, read August 2026. Two lines quote a self-pay care plan that FORM states excludes the medication. The other two quote an access membership, with the drug obtained through NovoCare or LillyDirect. In all four cases the number on the page buys the service, not the treatment.

  1. Who writes the prescription, and what do they see?

    The $95 buys the clinical relationship. A prescriber reviews the request and writes the prescription. The manufacturer's channel fills it. FORM names those channels, which is more than most services that route a prescription do.

    Two service shapes sit on one site. One is a care plan, one is an access membership. Both leave the medication to a third party and neither states its cost. That is a consistent model, and it is disclosed rather than buried. It also means the site publishes no price a buyer can budget from.

  2. What happens when the dose goes up?

    Neither shape states what happens as the dose climbs. The manufacturer sets that, not FORM. So the answer is not FORM's to give — but the pages do not say where to find it either.
