Who writes the prescription, and what do they see?

The patient answers a questionnaire and books a video visit. The card is held about 48 hours before the appointment. It is charged only if the doctor approves. Medication ships in 24 to 48 hours. Refills and follow-ups stay online. There is no subscription; the patient pays per reorder.
What happens when the dose goes up?
